Q1, Has your organisation undertaken asbestos surveys of all properties built before 1999? – (where your organisation is responsible for the management & maintenance of the premises)	Y	N

Q2 Has your organisation compiled an asbestos register of all asbestos containing materials (ACM’s) identified from the survey of premises built prior to 1999?	Y	N

Q3 Does your organisation have asbestos management plans in place for ongoing management of ACM’s in situ and is an appointed person assigned to ensuring these plans are carried out in full?	Y	N

Q4 Are all asbestos registers audited regularly (1 – 2 yrs) by appropriately qualified and competent personnel to ensure any ACM’s remain in safe condition?	Y	N

Q5 Do you ensure that prior to any major refurbishment or demolition a Category 3 asbestos survey is conducted to ensure ACM’s that may not have been accessed as part of a Category 2 general management survey are identified?

Q6 Have all relevant personnel likely to work in proximity to asbestos undergone asbestos awareness training in accordance with the Control of Asbestos at Work Regulations? (Personnel should include engineers, maintenance personnel, electricians, Cable & Telecommunications, Plumbers, Cleaners etc.)

Q7 If your organisation conducts internal audits or Surveys of ACM’s present within properties, have you ensured that the organisation has adequate insurance to protect itself in the event of any claims or incidents whereby the results of the audit or survey may be brought into question?

Q8 Have all managers who have a responsibility under the organisations asbestos management plans had appropriate asbestos management training?

Q9 Does your organisation have communication procedures in place to enable relevant information relating to location of ACM’s, emergency procedures etc to be relayed to staff and contractors on site?

Q10 Have your asbestos surveys been conducted by an organisation or surveyor appropriately accredited as a UKAS Inspection body or NIACS certified surveyor?

Q11 Where asbestos abatement work is being conducted on notifiable products does the appointed contractor hold an appropriate HSE licence and adequate insurance, where a consultant has been appointed to overview works and that consultant is authorised to influence the works or the planning of the works does the consultant hold an appropriate HSE supervisory licence?
